Patient Guide

Welcome to Creighton Medical Associates Dundee Clinic. Thank you for entrusting us with your health care. We are proud to offer excellent physicians, and a professional support staff committed to providing patients with medical care delivered with a personal touch.

Our clinics are designed with comfort, privacy and professionalism in mind. At Creighton Medical Associates, our goal is clear: we strive to exceed your expectations, so please don’t hesitate to let us know if there is anything we can do to make your visit more enjoyable. Your health and wellness is our greatest concern.

Creighton Medical Associates offers primary and specialty outpatient services at clinics located throughout the Omaha metro area. Our physicians and staff are proud to provide quality patient care and clinical research opportunities while giving educational support to the Creighton University School of Medicine.

Because we are a teaching institution, you may be seen by a medical student or resident during the course of your care. Please be assured that a Creighton faculty physician provides on-site supervision at all times.

Scheduling Appointments

An appointment must be made for each individual or family member who wants to:

  • see a physician
  • see a nurse for any reason (lab draw, blood pressure check, weight check, etc.)

Appointments can be made by calling (402) 280-1185. For the widest selection of available dates and times, please make your appointment in advance, if possible. We make every effort to see patients at their scheduled times, but medical emergencies can cause unavoidable delays. Sometimes patients require a little extra time, but rest assured that you will get all the time you need during your scheduled appointment.

If you have any questions regarding covered services and/or if a particular physician is accepted by your insurance plan, please call the number on your insurance card prior to your visit.

If you have medical questions or are unsure whether you or a family member should see a doctor, please call to discuss your concerns with our nursing staff. If the clinic is busy, you may be asked to leave a message, but be assured that we will return your call as soon as possible. If your situation is an emergency, call 911 immediately.

Interpreter

Interpreter services are available. If an interpreter is needed, please make a request at least 24 hours prior to your appointment.

Emergency and Urgent Care

For chest pain or life threatening emergencies, call 911.

Most insurance companies require pre-authorization for non-life threatening emergency room visits. Call your insurance provider if you have specific questions about ER coverage.

Urgent Care During Clinic Hours
Clinic hours are Monday – Friday, 8 a.m. to 5 p.m. ER visits are seldom the best choice for urgent care during clinic hours. For best results, call us at (402) 280-1185 in the event of urgent medical needs. Clinic physicians and nurses will assess the situation to determine the best option for you (appointment, prescription, referral to urgent care or ER).

Urgent Care After Clinic Hours
Call (402) 280-1185 if you need urgent medical care after clinic hours. Our answering service will contact the physician on-call to get you the care you need as quickly as possible.

What to expect at your appointment

Upon arrival at our clinic, you will be asked to sign in, present your insurance card and sign a privacy form. The reception staff will ask you to verify your address and phone number. If your insurance provider requires a copayment, we request that you pay it at the time of your visit. Please inform the receptionist if your office visit is a result of an accident or injury.

Once you have registered, our receptionist will ask you to have a seat in the reception area. Every effort will be made to minimize waiting time. If you wait more than 15 minutes without being called to the exam room, please notify a member of our registration staff. Please remember to bring all of your current medicines to your appointments, and feel free to request needed refills when speaking to the nurse.

If you will be late, please call to learn if you can still be seen that day or if you will need to reschedule. You are welcome to arrive for your appointment early, but it is unlikely that you will be seen earlier than your scheduled appointment time.

If you cannot keep your appointment, please call us at least 24 hours in advance to cancel. This will allow us to give your appointment time to someone else who may desperately need it.

Walk-ins
Our clinic operates on an appointment basis to offer you the best, most efficient care possible. As a result, walk-in appointments are discouraged except under exceptional circumstances.

Medications and Refills

Our nursing and medical staff works closely with area pharmacies to refill your prescriptions quickly and accurately. The best way to update your prescription is to ask for a refill at the time of your office visit. If you find that your prescription has expired, ask your pharmacy to fax your current prescription to (402) 280-1779. This will provide us with the exact medication and dosage information we need to expedite the process. Please allow 24 to 48 hours for a prescription refill. 

Lab Results

Your lab results will be available to you once they have been reviewed by your doctor, in most cases, three working days. You will receive a postcard by mail if your results are in the normal range. If there is an abnormal result or if there is any anticipated change in your plan of care, you will receive a personal call from your physician. If you wish to have a copy of your lab results mailed to you, please ask during your clinic visit.

Insurance and Billing

Our patient billing staff is available Monday through Friday, 8 a.m. to 4:30 p.m., to assist you with any billing questions or concerns. The billing office number is (402)280-5858.

If you have any questions regarding covered services and/or if a particular physician is accepted by your insurance plan, please call the number on your insurance card prior to your visit.

Although you are responsible for your own medical bills, our staff will assist you by filing insurance claims on your behalf. We will also mail you a copy of your current statements. You are responsible for copayments, deductibles and non-covered services. Please call our billing office for assistance if you experience difficulty paying your bill, would like to make arrangements for a payment plan or if your insurance coverage changes.

Referrals

Our nursing staff is happy to prepare referrals required by your insurance company. Please allow at least 48 hours to enable us to properly organize your paperwork. If requesting referrals by phone, please have your insurance group and member numbers ready when you call. Because referrals are necessary to obtain coverage not provided by your primary care physician, plan to see your primary care physician at least once a year to discuss your ongoing care.
